Step 2. Hamilton James & Bruce’s Site Induction
There are several forms we request that you use to assist us in monitoring your health and safety in your place of work.
Site Induction Registration Form
This form should be completed on the first day of placement for every temporary or contracting assignment. You must complete this form on site with your supervisor, then fax the completed form to your Hamilton James & Bruce consultant.

Hazard Identification Form
If you identify a hazard, it is your responsibility to notify us. Please complete the Hazard Identification Form to notify Hamilton James & Bruce of any potential or existing hazards.
The form must be sent on to your dedicated Consultant, who will investigate the hazard and follow up with the client.
Please follow the Hazard Identification procedure, as outlined in Section 4 of the Candidate OH&S Handbook, at all times.
